Shifts will be 5 eight-hour shifts. Serves as patient/nurse/provider liaison in care pathway for the chest pain, ACS/AMI patient population: manages daily census list, oversees documentation compliance to standards of care and core measures, collaborates with clinical staff and providers to ensure evidence based standards of care are being followed and communicates with care team any follow up needs for patient course of care. Responsible for reporting out TCD scorecard metrics on overall program performance and facilitates performance improvement projects related to clinical performance variances with clinical teams and committees.  RN) Registered Nurse ~ Associate Degree, or Registered Nurse Diploma Centerpoint Medical Center is a 285+ bed hospital featuring state-of-the-art equipment and technology.
