Position OverviewWheeler Staffing Partners is seeking a compassionate and experienced
RN Case Manager to deliver high-quality, patient-centered care in a home health setting. This role is responsible for managing patient cases, coordinating care plans, and collaborating with interdisciplinary teams to ensure optimal outcomes.
This is a field-based position covering Norwood and surrounding areas, offering autonomy, flexibility, and the opportunity to make a meaningful impact in patients lives.
Schedule & Compensation Model- Full-Time schedule
- Paid Per Visit Model:
- Minimum: 28 visits per week
- Typical Range: 2840 visits per week
- Mileage reimbursement provided
- Annual compensation range is an estimate based on productivity
Key Responsibilities- Provide skilled nursing care to patients in their homes in accordance with physician orders
- Conduct comprehensive patient assessments and develop individualized care plans
- Coordinate care with physicians, therapists, and other healthcare professionals
- Monitor patient progress and adjust care plans as needed
- Educate patients and families on disease management, medications, and treatment plans
- Ensure accurate and timely documentation in compliance with regulatory standards
- Promote patient independence and improve overall quality of life
Qualifications- Graduate of an accredited school of professional nursing
- Active Registered Nurse (RN) license in the state of Massachusetts (required)
- Minimum 1 year of RN experience required
- Home health experience preferred but not required
- Strong clinical, organizational, and communication skills
- Ability to work independently and manage multiple patient cases
Certifications & Requirements- Current RN licensure in Massachusetts
- Current CPR certification
- Valid drivers license with reliable, insured transportation
- Ability to travel to patient homes within the assigned territory
