Position Overview

Seeking an experienced Registered Nurse (RN) to provide direct patient care in a field-based home health setting in Moline, IL and surrounding areas. This role focuses on delivering high-quality in-home nursing services, completing documentation, and managing patient care independently. The ideal candidate has home health experience, is proficient in HomeCare HomeBase, and is comfortable working autonomously.

Schedule & Work Details

  • Location: Moline, IL and surrounding areas
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Hour Guarantee: 32 hours
  • On-call/Weekend Requirement: Yes, participation in on-call rotation required
  • Pay Rate: $51.00 to $53.00 per hour
  • Mileage Reimbursement: Yes, between patient visits from first patient to last patient

Key Responsibilities

  • Provide direct skilled nursing care to patients in their homes
  • Perform comprehensive patient assessments and ongoing evaluations
  • Complete accurate and timely documentation using HomeCare HomeBase
  • Deliver care in accordance with individualized care plans and physician orders
  • Monitor patient conditions and communicate changes to appropriate team members
  • Educate patients and families on disease processes, medications, and care plans
  • Ensure compliance with agency policies and regulatory standards
  • Participate in on-call rotation and respond to patient needs as required

Required Qualifications

  • Active Registered Nurse (RN) license
  • Home health experience
  • Experience with HomeCare HomeBase (HCHB)
  • Strong clinical assessment and documentation skills
  • Ability to work independently and manage a caseload

  • OASIS experience and/or certification
  • Strong organizational and time management skills
  • Excellent communication and interpersonal abilities
  • Dependable and self-motivated work style
  • Ability to adapt to a fast-paced, field-based environment
