Compensation
- Salary: $78,000 – $88,000 annually

- Full-time home health visits
- Minimum productivity: 6 points per week

Responsibilities
- Perform skilled nursing visits in patients’ homes
- Conduct patient assessments and develop care plans
- Administer medications and treatments
- Provide wound care and patient education
- Coordinate care with physicians and interdisciplinary team
- Complete accurate and timely documentation
- Monitor patient progress and adjust care as needed
Qualifications
- Active Registered Nurse (RN) license
- Home Health experience preferred (not required)
- Valid driver’s license and reliable transportation
- Strong assessment and documentation skills
- Ability to work independently and manage time effectively
- Excellent communication and patient care skills
