Position Overview

Seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality hospice care to patients within a designated territory. This role focuses on delivering patient-centered care, coordinating interdisciplinary services, and supporting patients and families through end-of-life care. The ideal candidate is autonomous, detail-oriented, and skilled in critical thinking.

Schedule & Work Details

  • Location: Coverage within assigned territory, with a 50-mile on-call radius
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM, unless scheduled for on-call
  • On-call/Weekend Requirement: 3 to 5 days per month, which may include any day of the week
  • Weekly Hour Guarantee: 40 hours
  • Pay Rate: $51.00 to $53.00 per hour
  • Mileage Reimbursement: Yes, between patient visits (from first visit to last visit)

Key Responsibilities

  • Provide direct patient care in accordance with hospice standards and individualized care plans
  • Conduct comprehensive patient assessments and develop appropriate care plans
  • Attend and actively participate in Interdisciplinary Group (IDG) meetings
  • Complete and submit IDG documentation and weekly notes accurately and timely
  • Collaborate with physicians, social workers, and other care team members to ensure continuity of care
  • Educate patients and families on disease progression, symptom management, and end-of-life care
  • Respond to on-call needs as scheduled, including urgent patient care situations
  • Maintain compliance with agency policies, procedures, and regulatory requirements

Required Qualifications

  • Active Registered Nurse (RN) license
  • Basic Life Support (BLS) certification
  • Hospice experience preferred
  • Ability to work independently and manage a caseload effectively
  • Strong clinical assessment and documentation skills

Preferred Qualifications

  • Prior hospice or palliative care experience
  • Strong critical thinking and problem-solving abilities
  • Ability to work autonomously with minimal supervision
  • Excellent communication and interpersonal skills
  • Experience participating in interdisciplinary care planning
