Position Overview:
Seeking an experienced Registered Nurse to deliver patient-centered care in a home health setting. This role is responsible for completing comprehensive assessments, developing and managing plans of care, and ensuring high-quality outcomes through collaboration with patients, families, physicians, and the interdisciplinary care team. The ideal candidate brings strong OASIS experience and a commitment to clinical excellence.
Schedule & Work Details:
- Location: Field-based role serving the Eureka Springs area
- Schedule: 8:00 AM to 4:30 PM
- Weekly Hour Guarantee: 32 hours per week
- On-Call Requirements: On call approximately 1 time per month, with participation in weekend rotations and potential holiday coverage
- Mileage Reimbursement: Yes, reimbursed between patient visits
- Pay Rate: $51 to $53 per hour
Key Responsibilities:
- Initiate, develop, implement, and revise individualized plans of care in collaboration with physicians and the healthcare team
- Perform comprehensive patient assessments, including OASIS assessments and medication reconciliation
- Ensure patients meet home health eligibility and medical necessity guidelines per payer requirements
- Provide and coordinate high quality clinical care within scope of practice and infection control standards
- Supervise and provide direction to LPNs and home health aides, including completing required supervisory visits
- Communicate effectively with physicians, patients, families, and agency staff regarding patient care, scheduling, and changes in condition
- Coordinate visits, complete coding requests, and ensure accurate and timely documentation in accordance with agency and payer requirements
- Maintain compliance with documentation standards and sync clinical data in a timely manner per company policy
- Participate in performance improvement initiatives aimed at reducing hospitalizations, improving patient safety, and enhancing outcomes
- Support orientation and training of new staff and serve as a preceptor when needed
- Participate in survey readiness and adhere to agency utilization management processes
- Maintain professionalism by attending meetings, case conferences, and required trainings with punctuality and engagement
- Demonstrate flexibility in scheduling to meet patient care needs, including weekends and holidays
- Complete all additional duties as assigned
Required Qualifications:
- Active Registered Nurse (RN) license in the applicable state
- Home health experience required
- OASIS experience required
- Strong clinical assessment and care planning skills
- Ability to work independently and manage a field-based schedule
- Reliable transportation and ability to travel within the service area
Preferred Qualifications:
- Experience with Homecare Homebase (HCHB) EMR system
- Strong communication, organization, and time management skills
- Leadership experience or ability to supervise clinical staff
Required Certifications:
- Current RN license
- Current CPR certification
