Position Overview:
We are seeking a highly skilled and organized Home Health Registered Nurse (RN) to provide comprehensive, in-home patient care across the Danville, Virginia service area. This position focuses on delivering skilled nursing services, completing Start of Care and OASIS assessments, and managing individualized patient care plans to support positive health outcomes.

Schedule & Work Details:

  • Location: Danville City, Pittsylvania County, and limited coverage in Halifax County
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Hours: 40 hours per week
  • Productivity Model: Pay-Per-Visit with expectation of 30 points per week
  • On-Call/Weekend Requirement: 1 full week per month
  • Mileage Reimbursement: Yes, reimbursed between patient visits (mileage begins after first patient and ends at last patient; travel from/to home is not reimbursed unless stopping at the office first)
  • Pay Rate: $51 to $53 per hour

Key Responsibilities:

  • Perform Start of Care (SOC) visits and comprehensive patient assessments
  • Complete OASIS documentation accurately and in a timely manner
  • Provide skilled nursing care in the home setting, including routine and follow-up visits
  • Develop, implement, and update individualized Plans of Care (POC)
  • Deliver hands-on patient care including wound care, medication management, and chronic disease management
  • Collaborate with physicians and interdisciplinary team members to ensure coordinated care
  • Educate patients and caregivers on treatment plans, disease management, and home care techniques
  • Maintain accurate and compliant clinical documentation using Homecare Homebase (HCHB)
  • Manage time effectively to meet productivity and visit expectations

Required Qualifications:

  • Active Registered Nurse (RN) license
  • Proven experience in skilled home health care
  • OASIS documentation experience within a home health setting
  • Proficiency with Homecare Homebase (HCHB)
  • Strong clinical assessment and critical thinking skills
  • Valid driver’s license and reliable transportation
  • Ability to manage a patient caseload independently

Preferred Qualifications:

  • Strong wound care experience, including compression wraps and wound vacs
  • Excellent time management and organizational skills
  • Team-oriented mindset with strong collaboration skills
  • Effective communication and patient education abilities
  • Experience working in a fast-paced home health environment
