Registered Nurse – Home Health Care (Contract)
Location: York, Maine Contract Length: 13 Weeks Schedule: Full-Time | 40 Hours/Week | 5 x 8-Hour Shifts Weekend Rotation Required (2–3 weekends/month)
Position Overview
"Navitas Healthcare, LLC" is seeking an experienced and compassionate Registered Nurse (RN) for a Home Health Care contract assignment in York, Maine. This opportunity is ideal for a skilled nursing professional with prior home health experience and strong knowledge of OASIS assessments.
The RN will provide high-quality patient-centered care in home settings while collaborating closely with interdisciplinary healthcare teams to support patient recovery, independence, and wellness.
Key Responsibilities
Deliver skilled nursing care to patients in home health environments
Complete and document OASIS assessments accurately and efficiently
Monitor patient conditions and report significant changes to the care team
Administer medications and prescribed treatments
Educate patients and families on care plans, medications, and health management
Maintain timely and accurate clinical documentation and timekeeping records
Qualifications
Active Registered Nurse (RN) license in the State of Maine
Current BLS/BCLS certification
Minimum 1 year of recent Home Health RN experience required
OASIS assessment experience required
Excellent communication and patient care skills
Ability to work independently in a home care setting
