Position Overview
A growing home health organization is seeking a Registered Nurse to provide skilled nursing services to patients in their homes. This role offers the opportunity to work one-on-one with patients, develop meaningful clinical relationships, and deliver personalized care in a home-based setting.
Schedule:
  • Full-time, 40 hours per week
  • Day shift: 8:30 AM - 5:00 PM
  • Every fourth weekend
  • Two holidays out of six annually
  • No on-call responsibilities
Key Responsibilities
  • Conduct comprehensive patient assessments and develop individualized plans of care.
  • Deliver skilled nursing services in accordance with physician orders, clinical standards, and regulatory requirements.
  • Monitor patient progress and adjust care plans as needed based on changes in condition.
  • Perform medication reconciliation and collaborate with providers regarding treatment plans.
  • Ensure timely and accurate clinical documentation within established organizational guidelines.
  • Provide patient and caregiver education to support treatment goals and promote positive outcomes.
  • Coordinate care with physicians, therapists, home health aides, and other interdisciplinary team members.
  • Manage visit schedules and maintain compliance with payer authorization requirements.
  • Communicate patient status updates and care plan changes to appropriate healthcare team members.
  • Supervise home health aide staff and complete required supervisory visits.
  • Maintain current knowledge of nursing standards, best practices, and regulatory requirements.
  • Meet productivity expectations and complete assignments independently and accurately.
  • Perform additional duties as assigned.
Required Qualifications
  • Associate Degree in Nursing or Nursing Diploma.
  • Minimum of six (6) months of Registered Nurse experience OR at least one (1) year of Licensed Practical Nurse experience.
  • Experience using computers and electronic documentation systems.
  • Active New Jersey Registered Nurse (RN) license.
  • American Heart Association (AHA) Basic Life Support (BLS) for Healthcare Providers certification.
Preferred Qualifications
  • Bachelor of Science in Nursing (BSN).
  • Home health or hospice nursing experience.
  • IV Certification.
