Job Summary

Penn Medicine Home Health is seeking an experienced Registered Nurse (RN) Case Manager to join the Princeton Health South Team, serving patients throughout Plainsboro and West Windsor, NJ. This position is ideal for an RN with a strong background in home health case management, OASIS documentation, and coordinating comprehensive patient care in the home setting. The RN will manage a high-volume caseload, providing skilled nursing care while ensuring accurate and timely documentation.

Responsibilities

  • Manage and coordinate patient care as the primary RN Case Manager in a home health setting.
  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Complete accurate OASIS assessments (SOC, ROC, Recertification, Discharge, etc.).
  • Provide skilled nursing care and patient/family education in the home.
  • Coordinate care with physicians, therapists, social workers, and interdisciplinary team members.
  • Monitor patient progress and adjust care plans as needed.
  • Perform wound assessments and provide wound care treatment when appropriate.
  • Maintain timely and accurate documentation in the EMR.
  • Manage a daily caseload of approximately 5–6 patient visits while completing high-volume documentation.
  • Ensure compliance with Penn Medicine policies, CMS regulations, and home health standards.

Required Qualifications

  • Active Registered Nurse (RN) license in New Jersey.
  • Minimum 2 years of recent Home Health RN experience.
  • Case Management experience is required.
  • Strong OASIS documentation experience is required.
  • Current BLS Certification (AHA preferred).
  • Valid driver's license, reliable transportation, and auto insurance.
  • Excellent communication, assessment, and organizational skills.
  • Ability to independently manage a high-volume patient caseload.

Preferred Qualifications

  • Wound Care experience preferred.
  • Experience working in a fast-paced, high-volume home health environment.
  • Familiarity with home health EMR systems (Epic experience is a plus).

Patient Volume

  • Approximately 5–6 patient visits per day
