Job Description: Registered Nurse (RN)
Position Overview:
All Embracing Home Care is seeking a compassionate and dependable Registered Nurse (RN) to provide high-quality, patient-centered care in a home setting. This role focuses on promoting patient independence, improving health outcomes, and delivering personalized care tailored to each individual’s needs. The RN will also play a key role in supporting and training Care Managers through medication practicum and clinical guidance.
Key Responsibilities:
- Perform initial and ongoing comprehensive patient assessments
- Develop, implement, and revise individualized care plans
- Provide skilled nursing care, including medication administration and treatments
- Monitor patient progress and document clinical findings accurately
- Identify changes in patient condition and communicate with physicians and care teams
- Educate patients and family members on disease management, medications, and care techniques
- Conduct medication practicum and competency training for Care Managers, ensuring safe medication administration practices
- Provide clinical oversight, guidance, and support to Care Managers as needed
- Ensure compliance with all healthcare regulations and agency policies
- Coordinate care with other healthcare professionals and support staff
Qualifications:
- Active Registered Nurse (RN) license in ND
- Previous nursing experience required; home care or community health experience preferred
- Experience in training or mentoring staff is highly desirable
- Strong clinical judgment and critical thinking skills
- Excellent communication, teaching, and patient education abilities
- Ability to work independently and manage a flexible schedule
- Valid driver’s license and reliable transportation
Work Environment:
- Provides care in patients’ homes within assigned service area
- Requires travel between patient locations
