Registered Nurse - Full Time/Part Time - Home Care

Job Summary
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our growing home care team. This full-time/part time RN position focuses on completing client assessments and reassessments to ensure high-quality, individualized care for patients in the Northern Virginia communities.


Responsibilities 

• Conduct initial client assessments and routine reassessments in client home settings
• Conduct hospital discharge assessments in client home settings
• Develop and update individualized care plans
• Ensure compliance with care standards

Qualifications

•  Must hold a current, unrestricted Registered Nurse (RN) license in the Commonwealth of Virginia
• Must have experience conducting in-home clinical assessments and completing Medicaid DMAS Forms 97, 98, 99, and 100
• Ability to work independently
