    Registered Nurse (RN)

    Weekends - Saturday & Sunday 8-10 visits per day

     

    We are looking for an experienced Registered Nurse (RN) to join our home health team. In this role, you will oversee patient care, support caregivers, ensure quality care, and help coordinate services for patients in their homes.

    Responsibilities

    • Visit patients in their homes and monitor the quality of care.
    • Assess patient needs and help develop appropriate plans of care.
    • Support and guide caregivers and clinical staff.
    • Communicate with patients, families, physicians, and the care team.
    • Ensure all documentation is accurate, complete, and submitted on time.
    • Identify and resolve patient or staff concerns.
    • Follow company policies and state and federal regulations.
    • Use sound nursing judgment to solve problems and improve patient outcomes.

    • Geriatrics

     

    Qualifications

    • Active Indiana Registered Nurse (RN) license.
    • At least 2 years of home health nursing experience (required).
    • Valid driver's license and reliable transportation.
    • Strong assessment, communication, and organizational skills.
    • Experience using electronic medical records (EMR/EHR).
    • Bilingual candidates are encouraged to apply.
