Job Summary

Position Summary

The Transfer Center RN Clinical Coordinator coordinates a patient-centered admission process for patients transferring into the Abrazo Community Health Network/Carondelet System ensuring placement in an appropriate level of care bed and financial screening per EMTALA regulations .The RN Clinical Coordinator employs critical thinking skills to expedite admissions into ACHN/Carondelet while demonstrating ACHN/Carondelet's Mission, Vision, Values and Service Standards in all interactions with referring facilities, physicians, employees and guests.

Responsibilities

  • Facilitates patient centric critical and emergent transfers into multiple hospitals comprising the Abrazo Community Health Network (ACHN) and Carondelet System utilizing service line specific transfer protocols.
  • Systematically employs critical thinking triage skills to assist sending facilities to transfer their patients to a higher level of care focusing on EMTALA guidelines.
  • Collaborates with multiple members of the sending and receiving healthcare teams to ensure a timely admission into the right bed in the right level of care with the correctly identified admission status.
  • Escalates situations falling outside of transfer protocol parameters and presents succinct SBAR to physicians and/or AOC.
  • Accurately documents all transfer and admission data.
  • Coordinates air or ground transportation according to patient acuity level and availability.
  • Maintains competence in all telephone, computer, image sharing, telemedicine, and other systems utilized in the transfer center.
  • Facilitates patient outbound calls while vetting appropriateness
  • Other related job tasks or responsibilities as assigned

Qualifications

  • Special Skills:

  • Organizational and leadership skills

  • Ability to decipher specific messages with background noise

  • Effective communication skills - phone etiquette, written and verbal - with all levels of staff, physicians, patients and families

  • Experienced at multi -tasking

  • Ability to present self in positive and professional manner, exhibiting confidence, tact, and enthusiasm

  • Must be sensitive to diverse cultural and bilingual issues

  • Adept at interpreting and analyzing qualitative and quantitative data

  • Ability to quickly triage, critically think in high pressure situations and prioritize

  • Minimum Requirements

  • Education:

  • Graduation from accredited nursing program

  • EMTALA Training through Tenet Health stream within 30 days of hire.

  • Experience:

  • Minimum of 5 years continuous clinical experience in a critical/acute care setting with adult neuro/trauma/cardiovascular/critical care or emergency room nursing preferred.

  • Ability to manage initial assessment data and prioritize patient care with appropriate decision making.

  • Licensure/Certifications:

  • Registered Nurse with current state licensure.

  • Current BLS certification required

  • Preferred Requirements

  • Education: BSN preferred

  • Experience: 8 years of critical care, emergency room, or house supervisor experience

  • Certification/Registration: ACLS training preferred.
