Job Requirements
  • Education Requirements: Associate or Bachelor’s in Nursing
  • Certifications Requirements: Active RN license WI required, BLS required, any additional certifications welcomed
  • Experience Requirements: 1+ year of experience required, previous travel experience preferred, strong clinical assessment skills, wound care, medication management, OASIS documentation experience preferred, ability to work independently in the field

Job Responsibilities
  • Provide comprehensive nursing care to patients in their home environment, including assessment, wound care, medication management, and patient education.
  • Document patient interactions accurately using Epic EMR system, ensuring compliance with all regulatory standards.
  • Collaborate with physicians, hospitalists, and interdisciplinary team members through standard communication pathways to coordinate patient care.
  • Manage caseload effectively, prioritizing visits to meet patient needs and maintain productivity goals of 5–6 visits per day.
  • Support floating or assisting other teams within the designated coverage area as needed, demonstrating flexibility and teamwork.
