Job Responsibilities:
  • Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
  •  Utilizing the state tracking system HHH (EMR like tool used by DPH) after receiving comprehensive training.
  • Completing assessments for clients within 24 hours of their family's arrival into the system.
  • Providing on-site medical support as needed by CSR ((Clinical and Safety Risk Assessment) families.
  • Assisting CSR families with medical referrals, appointment setup, and access to required medical services.
  • Educate patients and their families on health maintenance and disease prevention.
  • Ensure accurate documentation and compliance with all regulatory guidelines.
Requirements:
  • Active RN license in the state of practice.
  • Previous experience on patient admission and doing assessment.
  • Exceptional interpersonal and communication skills.
  • Committed to delivering high-quality, patient-centered care.
