SUMMARY OF POSITION
Responsible to coordinate interdisciplinary professional care Vascular Surgery patients as determined by individualized plan of care and clinical pathways. Functions include but not limited to:
• RN triage patient phone calls and follow up care coordination • Patient intake • Insurance authorization • Medical record reviews
The expectation is that the RN will report to various clinics to assist the attending with wound care, patient teaching, and coordination of follow up care. This is an excellent opportunity for an individual with vascular knowledge who enjoys supporting patients throughout their care journeys.
MINIMUM QUALIFICATIONS
• Two years nursing experience • Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact. • AHA BLS prior to end of orientation period
PREFERRED QUALIFICATIONS
• Experience as a case manager in discharge planning or insurance issues preferred. • Appropriate certification for specialty area preferred. • Proficiency in Microsoft Office preferred. • Motivational interviewing skills preferred.
