Department: Pace Clinic

Schedule: Day | Full-Time

Salary range: $31.91 - $45.05 per hour

How you'll make an impact in this role:
  • Deliver high-quality, direct nursing care to PACE participants, including acute triage, blood draws, wound care, and participation in an after-hours/weekend on-call rotation.
  • Collaborate with the Interdisciplinary Team (IDT) to develop, implement, and evaluate individualized care plans, ensuring timely updates based on participant needs.
  • Participate in IDT care plan and participant/family meetings, and perform assessments across care settings such as the PACE clinic, Adult Day Center, and participant residences.
  • Educate participants, caregivers, and staff on health management and end-of-life care, with flexibility to travel to homes, long-term care, and assisted living facilities as needed.
What minimum qualifications you'll need:

Licensure / Certification / Registration:

 

Required Credential(s):

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
  • Registered Nurse obtained prior to hire date or job transfer date.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
What additional requirements you'll need:

One or more years of RN experience.

Home Health experience a plus.
