How you’ll make an impact in this role


+ Create comprehensive care plans that span the entire continuum, ensuring that a patient’s unique values and family preferences are at the heart of every clinical decision.

+ Serve as the primary champion for your patients, proactively identifying and removing barriers to care and securing the essential resources they need to thrive.

+ Act as a vital liaison, coordinating between the hospital and post-acute services, ensuring that the hand-off is safe, clear, and supportive.

+ Use your expertise to guide medical record documentation and provide level-of-care recommendations that align treatment options with the best possible outcomes.

+ Advocate for the efficient and effective use of clinical resources, balancing high-quality care with smart operational stewardship.


What minimum requirements you’ll need


Licensure / Certification / Registration:


+ Licensed Registered Nurse credentialed from the Kansas Board of Nursing obtained prior to hire date or job transfer date required.

+ Case Manager credentialed from the American Case Management Association (ACMA) preferred.

+ Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.


Education:


+ Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.


What additional preferences we're seeking


+ Experience in Case Management, Discharge Planning, or Utilization Review.

+ Inpatient clinical experience.
