Position Overview:
Seeking an experienced Registered Nurse Case Manager to deliver high-quality, patient-centered care in a home health setting. This role is responsible for managing a caseload, completing OASIS assessments, coordinating care, and providing skilled nursing services including IV therapy and wound care. The ideal candidate has strong home health experience and is comfortable meeting productivity expectations in a field-based environment.

Schedule & Work Details:

  • Location: Field-based role serving Lorain, Lake, and Cuyahoga counties
  • Schedule: 8:00 AM to 4:30 PM
  • Weekly Hour Guarantee: 36 hours
  • Productivity Expectation: 30 points per week
  • On-Call Requirements: One weeknight rotation and one weekend per month
  • Mileage Reimbursement: Yes, reimbursed between patient visits
  • Pay Rate: $51 to $53 per hour

Key Responsibilities:

  • Manage a caseload of home health patients, ensuring delivery of high-quality, coordinated care
  • Complete OASIS assessments including Start of Care, Recertifications, and Discharges
  • Perform comprehensive patient assessments and develop individualized care plans
  • Provide skilled nursing care including IV therapy, wound care, and wound vac management
  • Educate patients and caregivers on disease processes, medications, and care plans
  • Coordinate care with physicians and interdisciplinary team members to optimize patient outcomes
  • Maintain accurate and timely clinical documentation in compliance with regulatory and agency standards
  • Meet productivity expectations and maintain efficient scheduling of patient visits
  • Participate in on-call rotation and respond to patient care needs as required

Required Qualifications:

  • Active Registered Nurse (RN) license in the applicable state
  • Home health experience required
  • Strong OASIS knowledge and experience
  • Proficiency in clinical assessment, care planning, and patient education
  • Ability to work independently in a field-based environment
  • Reliable transportation and ability to travel within assigned coverage areas

Preferred Qualifications:

  • Experience with Homecare Homebase (HCHB) documentation system
  • Strong organizational and time management skills
  • Experience with advanced skilled nursing procedures such as IV therapy and wound vacs
