Position Overview
We are seeking an experienced Registered Nurse (RN) Case Manager with strong hospital-based case management experience. The ideal candidate will have a minimum of 5 years of hospital case management experience and proficiency in Cerner/PowerChart and discharge planning platforms.
Key Responsibilities
Manage a caseload of approximately 25 28 patients.
Perform comprehensive patient assessments and develop individualized discharge plans.
Coordinate care across interdisciplinary teams to ensure appropriate resource utilization and timely discharges.
Facilitate communication between physicians, patients, families, and post-acute care providers.
Complete PASRR (ASCEND) entries and ensure compliance with state and federal regulations.
Utilize Cerner/PowerChart and WellSky (formerly NaviHealth) for documentation and care coordination.
Apply PAE knowledge and ensure accurate documentation.
Float within scope of practice as needed to meet facility requirements.
Requirements
Experience: Minimum 5 years of hospital case management experience (Required)
EMR: Cerner/PowerChart experience (Required)
Experience with WellSky (formerly NaviHealth) (Required)
PASRR entry experience (ASCEND)
Knowledge of PAE processes
Active RN license (Pending license not accepted)
First-time travelers will be reviewed
Local candidates considered on a case-by-case basis
