The Registered Nurse Case Manager is a community‑based role that provides health advocacy and support to clients throughout Lehigh and Northampton Counties. The Registered Nurse Case Manager helps individuals access the physical and behavioral health care they need by coordinating appointments, supporting hospital transitions, arranging transportation, and creating personalized plans that promote wellness and collaboration.
Responsibilities include, but are not limited to, the following:
Provide integrated service coordination for assigned consumers, collaborate with behavioral health navigators, engage internal/external stakeholders, and communicate with all healthcare staff to ensure holistic and necessary care.
Assume primary responsibility for developing, writing, implementing, evaluating, and revising treatment goals and plans in collaboration with the consumer and their identified supports.
Provide initial and ongoing assessment of mental health, physical health, and substance use; review labs, physician summaries, health data, medication lists, and ensure metabolic screenings are completed.
Support individuals in managing medication concerns, monitor medication changes, and provide behavioral and physical health education regarding their conditions.
Conduct ongoing assessment of gaps in care and complete referrals as needed.
This position is Community-based and requires travel to homes, and other community locations to provide services and support clients. Mileage reimbursement provided.
Earn $34.66 per hour.
You will receive Rate increases at 6 months and 12 months in this position!
