Registered Nurse Case Manager – Hospice
Location: Elmhurst, IL Schedule: Monday–Friday, 8 AM–5 PM (daily start/end times may vary; availability required between 8 AM–5 PM) On‑Call:
- One (1) 24‑hour weekend day per month
- Two (2) weekday on‑call shifts per month (5 PM–8 AM)
- Two (2) holidays per year – one summer, one winter, plus three floating holidays Travel: Local travel required across multiple counties
Overview
This is an opportunity for a dedicated Registered Nurse Case Manager to join our client and make a meaningful impact in the lives of patients receiving hospice care. You will be part of a supportive, mission‑driven team, providing essential care coordination, clinical leadership, and compassionate support to patients and their families during end‑of‑life care. Key Responsibilities
- Manage and oversee patient care within hospice settings, ensuring individualized, high‑quality care
- Develop, implement, and evaluate patient‑specific care plans
- Coordinate with interdisciplinary healthcare teams, including physicians, social workers, and support staff
- Provide leadership, mentorship, and guidance to nursing staff
- Maintain accurate, timely clinical documentation
- Communicate effectively with patients, families, and caregivers
- Participate in scheduled on‑call rotations, including weekend and weekday coverage
- Travel locally to patient homes and facilities across multiple counties
On‑Call Structure Details
- One 24‑hour weekend day per month:
- 8 AM–5 PM: scheduled field visits
- 5 PM–8 AM: true on‑call coverage
- Dedicated After‑Hours RN team support is available nightly (three RNs on staff each night)
- Additional stipends and compensation provided for on‑call fulfillment
- Two weekday on‑call shifts per month:
Required Qualifications
- Education: High School Diploma or GED
- Experience:
- Hospice experience (2+ years)
- Management or leadership experience (2+ years)
- 1+ years of experience in ANY of the following:
- Home Health Care
- Epic Home Health
- Home Health
- Licensure:
- Valid Registered Nurse (RN) license in the state of Illinois
- Certifications (ALL required):
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
- Other Requirements:
- Reliable transportation and ability to travel locally
- Strong time‑management and self‑organization skills
- Ability to work independently in field‑based environments
