Case Management, Certified Case Manager (CCM), Discharge Plans, Durable Medical Equipment, Needs Assessment, Patient Care, Physician Credential, Registered Nurse (RN)
Details: 13 week contract, Shift: M-F 5 x 8 Days 0830-1700
On Call/Weekend Requirement: No on-call requirement; traveler can pick up weekends if desired and available/they are needed
Requirements for the Position:- MD RN LICENSE
- AHA BLS
- 3 Years Minimum Experience Required
- Must have completed 1 prior travel assignment
- Experience with Care Coordination, Discharge planning, needs assessment/ Order DME.
- Acute Case Management exp required
Unit: Outpatient clinic, working in an acute hospital setting
Float Requirement: Float throughout the patient tower across the Med/Surg units as necessary
*MUST have MEDICAL INPATIENT EXPERIENCE*
Orientation: 07/14 or 07/28
