Graduate of an accredited school of Nursing (Bachelor's degree). Licensed as a Registered Nurse or obtain within a year in the State of Alaska. Current BLS or must be obtained within one year of hire and biannually thereafter.
A minimum of 6 months experience as a Case Manager or related clinical experience. Experience and sensitivity in working with diverse cultures. Ability to use computers, a phone systems, scanners, copiers, and other office equipment, trouble-shoot basic operational issues, and report problems to a supervisor willing and have the means to travel in large and small aircrafts, and have a valid Alaska driver's license.
Willing to take training and attend workshops and meetings periodically to enhance job performance and knowledge. Native preference given for qualified candidate.
Job Responsibilities
Provides case management support for all APIA Health Care Providers and Patients throughout APIA villages.
Key Responsibilities
- Provides support in acute, chronic, preventative, diabetes, and minor emergency medical services as appropriate.
- Ensure proper forms are accurately filled out and submitted to ensure patient care is delivered efficiently.
- Scans and/or enters patient data into the electronic health record; including lab results, medication reconciliation, determination of referrals, immunizations, and other information as it pertains to the patient and their history.
- Chart reviews to ensure all patient care is complete. Especially from specialty clinics to ensure all follow-up is completed and followed through.
- Coordinates patient travel via ANMC, Medicaid, and other systems as required for patient care.
Technical Skills
Utilize Electronic Health Systems: Oracle Vac TrAk, T.T.S., AFHCAN, Primary Care Dashboard, and other databases pertinent to patient care.
