Location: Santa Monica, CA
Duration: 13 Weeks
Shift: Day Shift, Monday–Friday, 5x8-Hour (8:00 AM – 4:30 PM)
Orientation: 40 hours (Non-billable)
Job Description
"Navitas Healthcare, LLC" is seeking an experienced Registered Nurse – Case Manager for an acute care hospital setting. The ideal candidate will have recent hospital-based case management experience and strong knowledge of utilization review and care coordination processes.
Key Responsibilities
- Perform patient case management and care coordination in an acute care setting
- Utilize InterQual criteria for utilization review and discharge planning decisions
- Collaborate with physicians, nurses, and interdisciplinary teams
- Coordinate patient admissions, transfers, and discharge planning
- Ensure appropriate resource utilization and length-of-stay management
- Maintain accurate documentation in EPIC EMR
- Communicate with insurance providers and external care agencies as needed
- Support continuity of care across the patient care continuum
Required Qualifications
- Active Registered Nurse license in California (or pending license accepted)
- Recent acute care hospital case management experience (must be within last 6 months)
- Strong experience with InterQual and EPIC systems (hard requirement)
- Minimum recent experience in case management role
- Strong clinical judgment and utilization review skills
- Excellent communication and coordination abilities
