Registered Nurse – Case Manager (Acute Care)

Location:Mission Hills, CA
Contract Duration: 13 Weeks
Compensation: $2,500 to $2,829 Weekly – Competitive Travel Pay Package
Shift: Day Shift | 8:00 AM – 4:30 PM
Schedule: 5x8s | Weekend Rotation Required | No Block Scheduling
Orientation: 40 Hours (Non-Billable)

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced Registered Nurse – Case Manager to support an acute care hospital case management team. The RN Case Manager will play a key role in care coordination, discharge planning, utilization review support, and interdisciplinary collaboration to ensure patients receive timely and appropriate care throughout hospitalization and transition planning.

This role is ideal for candidates with recent acute care hospital case management experience who thrive in a fast-paced healthcare environment and possess strong communication and organizational skills.

Key Responsibilities

  • Coordinate patient care and discharge planning within an acute care setting
  • Collaborate with physicians, nursing staff, social workers, and interdisciplinary teams
  • Perform utilization review and care coordination activities
  • Assess patient needs and develop effective discharge plans
  • Facilitate transitions of care and arrange post-discharge services
  • Educate patients and families regarding discharge plans and available resources
  • Maintain accurate and timely documentation within hospital systems
  • Ensure compliance with hospital policies, regulatory standards, and case management protocols
  • Participate in weekend rotation coverage as needed
  • Manage assigned caseloads with approximately a 1:20 patient ratio
  • Support patient throughput and continuity of care initiatives

Required Qualifications

  • Active California Registered Nurse (RN) License required
  • Pending California license accepted; must be active prior to start date
  • Minimum 2 years of acute care hospital Case Management experience required
  • Recent experience with hospital-based care coordination and discharge planning required
  • Strong understanding of acute care case management workflows
  • Excellent communication, collaboration, and organizational skills
  • Ability to work effectively in a fast-paced hospital environment
  • First-time travelers accepted (clinical screening required)
  • Flexibility to accommodate weekend scheduling needs

Preferred Qualifications

  • Previous travel nursing experience in Case Management
  • Experience in high-volume acute care facilities
  • Strong interdisciplinary collaboration background
  • Knowledge of utilization review and discharge planning regulations
