Your future role at a glance:

Location: Indianapolis, IN

Facility: Ascension Medical Group Clinic

Department: Center of Healthy Aging

Schedule: Monday-Friday 8am-4:30pm

How you'll make an impact in this role:
  •  A geriatric clinic case manager coordinates care for older adults to ensure their medical, social and emotional needs are met. The case manager will support caregivers by providing resources and education to support care of dementia patients.
  • Comprehensive Assessments: Evaluate a patient’s physical, cognitive, emotional, and social functioning to identify deficits and possible needs before the first consultation appointment.
  • Care Planning & Monitoring: Develop adaptable, short- and long-term care plans, and regularly check in to adjust these plans as the patient's health and caregiver's need evolves.
  • System Navigation: Help families understand and access complex systems.
  • Patient Advocacy & Education: Ensure the patient's and caregiver's preferences are respected, advocate for their dignity, and educate caregivers on how to provide appropriate support.

What minimum qualifications you'll need:

Licensure / Certification / Registration:

  • Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

  • Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
