Responsibilities:
  • Leads and coordinates care management–related projects focused on patient flow, transitions of care, discharge planning, and population health for pediatric and women’s health patients with emphasis on safe, timely transitions of care.
  • Coordinates initiatives addressing high‑risk pregnancies, postpartum care, medically complex children, and neonatal/pediatric discharge planning.
  • Develops project plans, timelines, workflows, and deliverables to support organizational priorities and regulatory requirements.
  • Serves as a clinical resource to nursing, social work, physicians, and ancillary departments to promote consistent, evidence‑based care management practices.
  • Helps nursing to facilitate ongoing movement and successful transitions of care within reasonable GMLOS timelines.
  • Ensures projects and workflows align with CMS Conditions of Participation, Joint Commission standards, and hospital policies related to care coordination, discharge planning, and patient rights.
  • Assists with preparation for regulatory surveys, audits, and tracers by supporting documentation, workflow standardization, and staff education.
  • Participates in quality improvement initiatives focused on maternal and pediatric health, length of stay, readmission and patient experience and supports continuous improvement.
  • Contributes to daily rounds with clinical providers and care management staff to ensure early identification of discharge needs to reduce delays and support safe, effective transitions and improve patient outcomes across the continuum.
  • Promotes patient‑ and family‑centered care, addressing developmental, cultural, psychosocial, and health literacy needs of mothers, infants, children, and families.
  • Supports initiatives addressing social drivers of health, access to care, and health equity in maternal and pediatric populations
  • Tracks project outcomes related to length of stay, readmissions, safe discharges, patient experience, and compliance indicators.
  • Analyzes current-state workflows and supports the development of standardized processes to improve efficiency, safety, and consistency in care management practices.
  • Monitors daily workflow expectations to ensure both timely and quality assessment/ intervention including but not limited to the following:
    a. New Consults
    b. Initial Assessments
    c. Timely response to referrals
    d. Accurate Disposition/ Equipment based upon medical necessity criteria.
    e. Documented regular communication with providers, staff, patients, and family.
  • Applies performance improvement methodologies (e.g., PDSA, Lean principles) as appropriate.
  • Collaborates with CM leadership to improve workflows and meet key performance indicators consistently
  • Supports staff education and adoption of new workflows, tools, and best practices related to care coordination and discharge planning.
  • Provides guidance and support during change implementation to ensure successful integration into daily operations.
  • Covers care management staffing assignment when shortages occur to ensure adequate coverage as needed.
  • Maintains current RN licensure and practices in accordance with professional nursing standards, organizational policies, and ethical principles.
  • Demonstrates accountability, effective communication, and professionalism in all interactions.
  • Performs additional duties as needed to support Care Management, Quality, and organizational priorities.
  • Completes all mandatory department, educational and hospital requirements
  • Adheres to current Infection Control and Safety Standards
  • Regular and prompt attendance
  • Ability to work schedule as defined and overtime as required
  • Related duties as assigned
Additional Information:

Employees must be in a regular position, working 20 hours or more per week (.50 FTE or greater) to qualify for benefits.

Qualifications:
  • Bachelor's Degree in nursing from an accredited institution and 3 years of utilization review, case management, or related experience. Required
  • Master's Degree in nursing Preferred
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure in accordance with the Nurse Licensure Compact (NLC) for Alabama Upon Hire Required
  • Accredited Case Manager Preferred or
  • Certified Case Manager Preferred
  • Comparable combination of education and experience may substitute for the above requirements.



Knowledge, Skills, and Abilities

  • Acute Care Management/Discharge Planning experience preferred Acute Care Management/Discharge Planning experience preferred
  • Pediatric and/or Women's Health patient population experience preferred Pediatric and/or Women's Health patient population experience preferred
