Registered Nurse (RN) – IDD Host Home Program
Location: Austin, TX (Hybrid)
Salary: $75,000–$80,000 annually
Schedule: Monday–Friday, 8:00 AM–5:00 PM
This RN role supports our Host Home Program and focuses on case management, care coordination, and oversight rather than hands-on clinical care.
This position is ideal for candidates located in Pflugerville, Round Rock, Cedar Park, or Leander, where the caseload is based.
The role begins in-office for training and transitions to a hybrid schedule (remote work, field visits, and ~1 day/week in office)
Conduct quarterly and as-needed nursing assessments
Develop and monitor individualized health care plans
Coordinate care with providers, pharmacies, and support teams
Provide oversight and training to Host Home providers
Ensure physician orders are followed and documented
Complete regular home visits and maintain accurate documentation
Collaborate with the Program Director on care and compliance
Caseload located in North Austin and surrounding areas
Frequent local travel required (personal vehicle + mileage reimbursement)
Remote work after training, with field visits and limited in-office time
Must have reliable internet and be comfortable with electronic documentation
Must be comfortable working with individuals with intellectual disabilities
Active Texas RN license
1+ year experience with IDD population preferred
Strong assessment, communication, and organizational skills
Valid driver’s license, insurance, and reliable vehicle
CPR/First Aid certification
