Job Summary/Objective:
Assertive Community Treatment (ACT) is an evidenced based model (EBP). The ACT RN will maintain the EBP indicators, and ensure customers are living the most person-centered, self-determined life. The team provides basic services and supports essential to maintaining a customer’s ability to function in community settings, including assistance with accessing basic needs through available community resources, such as food, housing, and medical care and supports to allow customers to function in social, educational, and vocational settings. ACT services are based on the principles of recovery and person-centered practice and are individually tailored to meet the needs of the customer. Services are provided in the customer’s residence or other community locations by all members of the team.
Essential Duties and Functions:
Acts as a member of the Assertive Community Treatment (ACT) Team.
Oversees medication and provides direct services to the customers in the community.
Nursing services are utilized to ensure that customers receive the right medication dosage and frequency and document the justification for medications and outcomes as defined in the treatment plan.
Demonstrates the ability to accurately evaluate, assess, review and complete the customer’s annual RN assessments and health history.
Provides education about mental illness and coordinates medication services with primary care physicians.
Performs physician evaluations as scheduled. Establishes protocol for and follow up with customer medication refills.
Utilizes and monitors medications stored the ACT med room. Facilitates medication ordering through the Indigent Drug Program.
Qualifications:
Must be a licensed Registered Nurse within the State of Michigan.
Must obtain training on the Basics of ACT Programs and Principles, through ACT specific training resources within ninety (90) days of hire; and shall participate in annual training specific to ACT curriculum.
Must obtain and maintain CPR Certification.
Not excluded from participating in Medicaid, Medicare, or any other federal or Michigan health care programs.
No criminal history that would preclude employment under federal and state laws and regulations in a federal or state health program.
Must support a trauma informed culture of safety to aid customers in their recovery process.
Two (2) years’ experience in working with individuals experiencing chronic and persistent mental illness
Specialized training, skills, and abilities: Lived experiences with behavioral health issues desired
One (1) year of computer experience – Word, Excel and electronic medical record software preferred.
