What You'll Do

As our RN Administrator & Case Manager, you'll serve as a vital clinical leader, ensuring every client receives personalized, high-quality care while supporting caregivers, families, and the office team.

Your Responsibilities Include:

  • Conducting comprehensive client assessments and initial nursing evaluations
  • Developing, implementing, and updating individualized plans of care
  • Coordinating services to ensure seamless client support
  • Monitoring client progress and identifying changes in condition
  • Educating clients, families, and caregivers on care needs and best practices
  • Supervising and supporting caregivers to promote excellent client outcomes
  • Assisting with medication management and health monitoring
  • Reviewing and maintaining accurate clinical documentation
  • Preparing nursing notes, visit summaries, and care updates
  • Responding to urgent client, family, and caregiver concerns as needed
  • Ensuring compliance with all state nursing regulations and ComForCare policies
  • Serving as the clinical bridge between non-medical home care services and skilled nursing oversight

What We're Looking For

Required Qualifications

  • Graduate of an accredited school of professional nursing
  • Active, unrestricted RN license in the state of practice
  • 2–5 years of nursing experience
  • 2–5 years of case management experience
  • 2–5 years of administrative nursing experience
  • Strong clinical assessment and critical-thinking skills
  • Excellent communication and documentation abilities
  • Professional, compassionate, and client-focused approach
Preferred Qualifications

  • Experience in home care, assisted living, memory care, hospice, or senior services
  • Dementia care experience
  • Leadership or supervisory experience
  • Passion for serving older adults and their families
