Job Duties: Perform comprehensive nutrition screenings and assessments. Develop and implement individualized Medical
Nutrition Therapy(MNT) plans. Document nutrition assessments, interventions, and outcomes within the electronic medical
record. Provide nutrition groups, education and counseling to patients, families, and caregivers. Participate in facility-wide and
departmental Quality Assurance and Performance Improvement initiatives. Participate in multidisciplinary patient care rounds
and departmental meetings. Support staff development initiatives and contribute to improved clinical outcomes. Complete
Swallowing Screen as needed.
Minimum requirements, including licenses and certifications: Bachelor's or Master's Degree in Clinical Nutrition, Dietetics, or a
related field from an accredited institution. AND Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential.
Background check requirements: the agency provides their background check, however we also require the candidate to complete the SCR form, which we enter and run, and we also run the SEL-staff exclusion list. In tandem with the agency's
background check, together these will comprise the background requirements.
Vaccination requirements: Immunization Records (MMR) - (Titers if MMR's are older than 10 years), PPD (within the past year).
Proof of immunizations include MMR and TB through a recent skin test (PPD) are required; a Flu vaccine is preferred. When
you present the full array of candidate's immunization documentation, have it contained in one single pdf.
OMH required training (This training will be provided by OMH): HPC Orientation Training, TRUST Initial Training, and BLS/First
Aid.
