Position Summary:
This is a high-impact opportunity for a Wound Care focused Nurse Practitioner who is ready to take ownership of their practice in a mobile, relationship-driven care model. You will manage complex wounds, guide treatment plans, and serve as a key clinical partner across your assigned communities. This role offers independence, flexibility, and the ability to grow your expertise in both wound care and geriatric medicine.
Responsibilities:
• Provide on-site rounding, primary care, and advanced wound care services.
• Manage complex wounds including pressure injuries, ulcers, and post-acute conditions.
• Drive chronic care management and preventative care strategies.
• Collaborate with physicians, nurses, and interdisciplinary teams.
• Reduce hospitalizations through proactive clinical care.
• Communicate with families, caregivers, and facility staff.
• Complete documentation, medication management, and lab coordination.
Qualifications:
• Active Certified Nurse Practitioner (CNP) license in Minnesota (AGNP preferred).
• Experience in wound care, geriatrics, primary care, or post-acute care preferred.
• Valid driver’s license and ability to travel throughout the Twin Cities Metro.
• Strong ability to work independently in a field-based setting.
• Passion for delivering high-quality, patient-centered senior care.
