Under the guidance of the Population Health Manager, the Referral Specialist plays a vital role as part of a collaborative patient care team dedicated to delivering high-quality, efficient, and patient-centered services. This role involves managing all aspects of medical referral processes, including scheduling, documentation, and follow-up for both in-house and external providers. Additionally, the Referral Specialist provides day-to-day supervision, mentorship, and workflow oversight for the referral team, ensuring seamless coordination among healthcare professionals, patients, and community resources. Working alongside physicians, mid-level providers, nurses, and medical assistants, the Referral Specialist is committed to upholding the organization’s core values of Service, Dignity, Justice, and Excellence in all activities.

Key Responsibilities:

Patient Relations 
  • Engage cooperatively and compassionately with patients, healthcare providers, and community stakeholders.
  • Maintain strict patient confidentiality and adhere to HIPAA regulations.
  • Provide supportive communication and empathetic care to enhance patient experience.

Referral Management 
  • Obtain, process, and coordinate referrals from healthcare providers for in-house and external specialists or testing.
  • Ensure timely authorizations by tracking, following up, and escalating issues as needed.
  • Prepare and submit referral documentation accurately, including urgent and stat referrals.
  • Monitor referral status within electronic medical records (EMR), providing regular updates and ensuring closure of the referral loop.
  • Review consultation reports and coordinate follow-up actions in collaboration with providers.
  • Assist patients with appointment scheduling and address any barriers they encounter.
  • Manage retro-authorization requests and distinguish between primary care and specialty visits.

Provider & Team Support 
  • Participate actively in daily care team meetings and departmental discussions.
  • Serve as a primary contact for referral questions via phone, portal, or mail.
  • Coordinate logistics such as transportation to medical appointments.
  • Document all activities thoroughly in the EMR, complying with regulatory standards.
  • Act as a liaison between patients, providers, specialty care facilities, hospitals, and community organizations.

Additional Responsibilities:
  • Perform other administrative or operational tasks as assigned by the supervisor.

Qualifications:
  • Education: High school diploma or GED required.
  • Experience: Minimum of 3 years in healthcare, with at least 2 years in referral coordination, authorization, or related roles.
  • Skills & Abilities:
    • Strong communication skills, both verbal and written.
    • Ability to work independently, multitask, and thrive in a fast-paced environment.
    • Proficiency with EMR systems and knowledge of healthcare billing and coding (CPT, ICD-10).
    • Demonstrated leadership and mentorship capabilities.
    • Bilingual in English/Spanish preferred.
    • Reliable transportation and ability to lift up to 10 lbs.
