** MISSION**
Kheir's mission is to increase and provide access to culturally and linguistically-sensitive quality primary healthcare and human services support to the underserved and uninsured residents of Southern California.
** ASSIGNMENT SUMMARY**
The Referral Coordinator will be responsible for developing and implementing organized care management procedures and coordinating inbound and outbound referrals at Kheir Center. They will be responsible for maintaining current knowledge ofrelevant compliance requirements, clinic operations, and managed care programs. They will collaborate with the care team and clinic leadership to ensure efficiency and accuracy in referral processing. They will assist the PCC team as needed with coordination for urgent referral follow-ups for patients' specialty care and will troubleshoot patient care issues and complaints. This person must have exceptional organizational skills with the ability to prioritize their daily workload and handle complex systems and information. The person must also demonstrate ethical conduct and maintain the confidentiality of patient information. The employee must perform job duties with sensitivity to the patient's diverse cultural backgrounds. The coordinator may also perform clerical functions as needed. They must be highly proficient in Microsoft Office programs and other computer functions in order to maintain detailed records. This is an hourly position. Patient facing roles require bilingual proficiency in English/Korean or English/Spanish.

** WORKING CONDITIONS/PHYSICAL REQUIREMENTS:**
This is primarily an office position that requires only occasional bending, reaching, stooping, lifting and moving of office materials weighing 25 pounds or less. The position requires daily use of a company provided computer and requires entering, viewing, and revising text and graphics on the computer terminal and on paper. The position requires occasional travel to attend on- and off-site meetings.

Kheir Clinic (formally known as Kheir Center) is a nonprofit community clinic and Federally Qualified Health Center in Wilshire Center/Koreatown, located approximately 3 miles west of Downtown LA. We are committed to providing linguistically and culturally-sensitive quality primary healthcare and human services support to the underserved and uninsured residents of Southern California. Kheir Clinic was established in 1986 to serve as a bridge between the low-income, recently-immigrated, non-English speaking Korean community and local health and social service providers in LA. Today, Kheir is a broad-based agency and Federally Qualified Health Center (FQHC) that provides comprehensive services for underserved populations in four interrelated areas: Health Care, Human Services, Adult Day Health Care, and Affordable Housing. Kheir is currently the only Federally Qualified Health Center (FQHC) in the nation that offers a full suite of services in 5 different languages including: English, Bengali, Korean, Spanish and Thai. We currently have 8 sites and 150 employees with 15+ contract consultants. We are expected to provide over 123,000 patient visits this year.
