** MISSION**

Kheir's mission is to increase and provide access to culturally and linguistically-sensitive quality primary healthcare and human services support to the underserved and uninsured residents of Southern California.

** ASSIGNMENT SUMMARY**

The Referral Coordinator will be responsible for developing and implementing organized care management procedures and coordinating inbound and outbound referrals at Kheir Center. They will be responsible for maintaining current knowledge ofrelevant compliance requirements, clinic operations, and managed care programs. They will collaborate with the care team and clinic leadership to ensure efficiency and accuracy in referral processing. They will assist the PCC team as needed with coordination for urgent referral follow-ups for patients' specialty care and will troubleshoot patient care issues and complaints. This person must have exceptional organizational skills with the ability to prioritize their daily workload and handle complex systems and information. The person must also demonstrate ethical conduct and maintain the confidentiality of patient information. The employee must perform job duties with sensitivity to the patient's diverse cultural backgrounds. The coordinator may also perform clerical functions as needed. They must be highly proficient in Microsoft Office programs and other computer functions in order to maintain detailed records. This is an hourly position. Patient facing roles require bilingual proficiency in English/Korean or English/Spanish.

** MINIMUM QUALIFICATIONS**

  • Minimum High School diploma; Bachelor's Degree a plus but not required
  • 1 to 2 years of related experience in a fast-paced professional medical environment.
  • Ability to exercise discretion to ensure that patient confidentiality is maintained and a professional, respectful clinic/department environment exists at all times.
  • Familiarity with computer and Windows PC applications, which includes the ability to learn new and complex computer system applications
  • Excellent written and verbal communication skills
  • Excellent time management and organizational skills
  • Excellent computer skills, including email, Microsoft Office (Excel!) & data tracking
  • Ability to work quickly, follow directions and make independent decisions
  • Ability to maintain confidentiality when handling patient medical records
  • Experience in community clinic environment a plus
  • Bilingual English/Spanish or English/Korean required

** ESSENTIAL FUNCTIONS/RESPONSIBILITIES**

  • Serve as an in‐house expert & coordinator of all patient‐referral options, including
  • criteria, reporting, fees, etc.
  • Perform data entry and prepare tracking reports for both internal & external patients
  • Provide feedback to Care Team regarding process improvement.
  • Garner & maintain knowledge of Health Plan compliance requirements, medical group operations, and medical terminology
  • Coordinate both inbound & outbound referrals
  • Produce and update databases to track and report relevant transactions.
  • Assist the Referral Coordinators with closing the referral loop
  • Assist with audits of Quality Measure Requirements
  • Collaborate with the clinical staff to ensure high quality service
  • Work with appropriate staff to develop and implement corrective action plans or practice refinements for quality and process improvements.
  • Maintain patient care database and up-to-date communications
  • Manage all reporting requirements for referral‐related grants and contracts
  • Stay informed of all federal, state, and local regulations and grant requirements
  • Be flexible with schedule to attend various meetings.
  • Other duties as assigned & necessary

** WORKING CONDITIONS/PHYSICAL REQUIREMENTS:**
This is primarily an office position that requires only occasional bending, reaching, stooping, lifting and moving of office materials weighing 25 pounds or less. The position requires daily use of a company provided computer and requires entering, viewing, and revising text and graphics on the computer terminal and on paper. The position requires occasional travel to attend on- and off-site meetings.

Today, Kheir is a broad-based agency and Federally Qualified Health Center (FQHC) that provides comprehensive services for underserved populations in four interrelated areas: Health Care, Human Services, Adult Day Health Care, and Affordable Housing. Kheir is currently the only Federally Qualified Health Center (FQHC) in the nation that offers a full suite of services in 5 different languages including: English, Bengali, Korean, Spanish and Thai. We currently have 8 sites and 150 employees with 15+ contract consultants. We are expected to provide over 123,000 patient visits this year.
