The Referral Coordinator is responsible for managing and coordinating all internal and external patient referrals across service lines to ensure timely, accurate, and seamless access to care. This role plays a critical part in supporting a centralized referral management model that enhances patient experience, continuity of care, and operational efficiency.

Serving as a key liaison between patients, providers, internal care teams, and external partners, the Referral Coordinator ensures referrals are processed, tracked, and closed appropriately within the Electronic Health Record (EHR), in alignment with organizational standards and payer requirements.

Bilingual pay differential
High School diploma or GED required.
Medical Assistant (MA), Behavioral Health Technician (BHT), or equivalent clinical experience strongly preferred.
1-3 years of experience in a healthcare setting, preferably in referral coordination, scheduling, patient access, or care coordination.
Proficiency with EHR systems (NexGen preferred) and Microsoft Office applications
Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment.
Must have a valid Arizona driver's license, be 21 years of age with a minimum of 3 years driving experience, and meet requirements of Terros Health's driving policy
Must successfully pass a TB screening and criminal background check
