  • Contract Duration: 3 months
  • Start: May 25, 2026
  • Pay Rate: $27.00 $28.00 / Hour (USD)
  • Shift / Schedule: Sunday Thursday, 8:00 AM 4:30 PM
  • Work Authorization: Applicants must be authorized to work for ANY employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa at this time.
  • Submission Requirement: BILH coversheet required at time of submission.

Job Summary

The Case Manager has the responsibility for providing case management services to individuals who are detoxing from substances. This includes assessing service needs; counseling, developing plans for access to services, liaison with client, families, programs, and service providers.

Key Responsibilities

  • Responsible for maintaining individual case load, which involves conducting individual intake evaluations, treatment and aftercare planning, medication management support, and safety planning as needed.
  • Completes all documentation in a timely manner, as set forth by regulations.
  • Coordinate with collaterals and referral sources as needed.
  • Assist in retrieval of patient medications from local pharmacies.
  • Acts as a support to other staff in milieu management, de-escalation, and crisis management.
  • Coordinate and support transportation needs to necessary appointments and aftercare.
  • Maintain Virtual Gateway access and complete disenrollment assessments as required.
  • Engage in identifying and locating possible contraband on the unit, performing unit searches as needed.
  • Assess patients' continuing care needs, contacting appropriate aftercare resources and arranging for transfer or admission to appropriate programming.
  • Shares responsibility for education and group programming, including recreation and wellness.
  • Attends staff meetings and supervisory sessions, as scheduled.
  • When possible and/or requested, provide information for affected family members to self-help.
  • Other related duties as assigned.

Required Skills

  • Knowledge of relevant community resources for continuing care/aftercare placement.
  • Must demonstrate ability to relate to a substance abuse treatment population in a sensitive manner.
  • An interest and willingness to work in a multicultural setting and must share a customer service orientation.
  • Manages difficult or emotional client situations; responds promptly to client needs.
  • Focuses on solving conflict, not blaming; maintains confidentiality.
  • Listens and gets clarification; responds well to questions; demonstrates group presentation skills.
  • Writes clearly and informatively.
  • Ability to respond to common inquiries or complaints from consumers, regulatory agencies, or other service providers.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.

Education Requirements

  • Bachelor's Degree (BA) or equivalent; or Associates degree in Human Services field.
  • Two to three years related experience and/or training in the area of chemical dependency required.
  • Certified Addiction Counselor (CAC) preferred.
  • Must be able to complete on-site CPI and CPR training/certification.
