Adecco Healthcare & Life Sciences is currently hiring Rapid Access Coordinator I in Knoxville, TN.
This is an on-site, full-time position.

Pay: Starting at $17.00+ per hour (based on qualifications and experience)
Type: Temp-to-hire
Schedule: Day shift (7:00 AM - 7:00 PM), rotating weekends (initially every other weekend; subject to change as staffing expands)
Location: Knoxville, TN 37915

Primary Responsibilities:
• Provide skilled nursing services in the home health setting, including start of care admissions, comprehensive assessments, and ongoing patient management in accordance with physician orders
• Conduct patient assessments and develop individualized care plans based on clinical findings and patient needs
• Ensure accurate, timely, and compliant documentation of all patient care activities in Homecare Homebase (HCHB) EMR system
• Coordinate care with physicians and interdisciplinary healthcare team members to ensure continuity and quality of care
• Monitor patient conditions, identify changes in status, and implement appropriate nursing interventions
• Educate patients and caregivers on disease management, medications, and home care instructions

Qualifications:
• High school degree or GED
• At least two (2) years of experience in a health-related field required

Pay Details: $17.00 to $19.00 per hour
