QAPI Specialist (RN or PT)

Hillcrest Home & Community Services is currently seeking a clinically strong, detail-oriented professional who is passionate about improving outcomes, driving accountability, and partnering with operational leaders to elevate quality across the organization.

This role offers the opportunity to make a meaningful impact by leading quality initiatives, analyzing performance data, and supporting regulatory readiness in a collaborative, fast-paced environment.

Key Responsibilities:

  • Lead and support Quality Assurance & Performance Improvement (QAPI) programs
  • Analyze clinical and operational data to identify trends, risks, and opportunities
  • Partner with Directors of Nursing, Therapy, and operational leaders to drive improvement plans
  • Support survey readiness, audits, and regulatory compliance efforts
  • Track, trend, and report key quality metrics and outcomes
  • Provide education and guidance on quality improvement processes and best practices
  • Promote a culture of accountability, continuous improvement, and operational excellence

Qualifications:

  • Active Registered Nurse (RN) or Physical Therapist (PT) license required
  • 3+ years of experience in healthcare (long-term care, rehab, or post-acute preferred)
  • Experience with QAPI, quality improvement, infection control, or compliance strongly preferred
  • Strong analytical and problem-solving skills
  • Ability to interpret data and translate into actionable plans
  • Excellent communication and collaboration skills
  • Ability to work across multiple sites and teams
