JOB REQUIREMENTS: The Public Health Nurse focuses on prevention of
illness, injury or disability, the promotion of health, and maintenance
of the health of populations in accordance with standards, codes, and
regulations of the WI Department of Health Services and Winnebago
County. This position will focus primarily on communicable disease
prevention and control, infection control, epidemiology, immunizations,
STI's and STI teaching, HIV/AIDS, tuberculosis, lead poisoning
prevention, refugee health, influenza, blood-borne pathogens, syndromic
surveillance, and food/waterborne/vector disease outbreak investigation.
ESSENTIAL FUNCTIONS: 1. Plan and provide health assessment, programs,
projects, interventions, and/or strategies at the individual, family,
and/or community level (e.g., care management, primary care, education,
referral, and policy, systems, and environment change) that is
culturally appropriate to achieve relevant objectives and goals (e.g.,
understanding health prevention, disease management and community
resources and improving individual/community health) as needed and
appropriate. 2. Builds and maintain relationships with community
agencies and members who are committed to improving the health of the
community, serves as a member and possible facilitator of local
collaborative groups; participates as a member of local and state public
health committees. 3. Assist in conducting community population
assessments through the collection, monitoring and analyzing of health
data to help determine needs of the population and work with
governmental and community partners to help fill gaps and improve
policy, systems or the environment to improve health. * OTHER
EXPERIENCE AND QUALIFICATIONS: Bachelor's degree in nursing with one
year of public health nursing or general hospital experience is
preferred. Current WI license as a Registered Nurse. Current WI
