Primary Responsibilities<\/span><\/span><\/b>

  • Conduct in -depth psychiatric assessments and evaluations for residents within skilled nursing and long -term care facilities.<\/span>
    <\/span><\/span><\/p><\/li>

  • Identify mental health conditions and create individualized, evidence -based treatment plans tailored to each patient.<\/span>
    <\/span><\/span><\/p><\/li>

  • Manage and adjust psychiatric medications while monitoring patient progress and response.<\/span>
    <\/span><\/span><\/p><\/li>

  • Work collaboratively with an interdisciplinary care team—including psychologists, physicians, nurses, and social workers—to ensure coordinated and comprehensive treatment.<\/span>
    <\/span><\/span><\/p><\/li>

  • Provide therapeutic support, including counseling and psychotherapy, as appropriate.<\/span>
    <\/span><\/span><\/p><\/li>

  • Educate residents and their families about diagnoses, medication use, treatment goals, and strategies for maintaining mental wellness.<\/span>

  • Maintain accurate, timely, and compliant documentation within the electronic health record system.Participate in case discussions, interdisciplinary meetings, and quality improvement projects aimed at enhancing patient care<\/span>

    • Qualifications<\/span><\/span><\/b>

      • Active <\/span><\/span>New York State Nurse Practitioner license<\/span><\/span><\/b> with <\/span><\/span>PMHNP certification<\/span><\/span><\/b>.<\/span>
        <\/span><\/span><\/p><\/li>

      • Ability to work independently while effectively managing a varied caseload.<\/span>
        <\/span><\/span><\/p><\/li>

      • Strong clinical assessment, diagnostic, and medication management skills.<\/span>
        <\/span><\/span><\/p><\/li>

      • Excellent communication and interpersonal abilities with the capacity to build trust and rapport with patients and staff.<\/span>
        <\/span><\/span><\/p><\/li>

      • Proficiency in EHR systems and basic computer applications.<\/span>
        <\/span><\/span><\/p><\/li>

      • A compassionate, patient -centered approach to psychiatric care.<\/span>
