Description
ConnextCare is seeking a compassionate and experienced Psychiatric Nurse Practitioner (PMHNP) to join our integrated healthcare team. The Psychiatric Nurse Practitioner will provide high-quality, patient-centered psychiatric and behavioral healthcare services with an emphasis on whole-person wellness and community health.
Under the guidance of the Chief Medical Officer and collaborating physicians as appropriate, the PMHNP will work closely with medical providers, behavioral health clinicians, nursing staff, and care teams to ensure the highest standards of clinical quality, patient satisfaction, and continuity of care.
The ideal candidate will have experience delivering compassionate, evidence-based psychiatric care to both pediatric and adult populations and thrive in a collaborative, integrated healthcare environment.
Essential Duties & Responsibilities
- Provide direct psychiatric patient care including assessment, diagnosis, treatment, medication management, crisis intervention, and referrals
- Conduct therapy, psychiatric evaluations, medication assessments, and follow-up care for new and existing patients
- Develop individualized treatment and service plans based on patient assessments
- Deliver ongoing medication management services and therapeutic interventions
- Collaborate with physicians and interdisciplinary care teams to ensure coordinated patient care
- Maintain accurate, timely, and compliant medical documentation in the Electronic Health Record (EHR)
- Complete encounter forms and coding documentation accurately
- Participate in quality assurance activities, peer review, audits, and continuous quality improvement initiatives
- Provide consultation and behavioral health education to staff, caregivers, and patients
Requirements
Required Education & Licensure
- Current New York State Registered Nurse (RN) license
- New York State Certified Psychiatric Nurse Practitioner (PMHNP)
- Master's Degree in Psychiatric Nursing or related Nurse Practitioner program required
Required Skills & Experience
- Experience providing psychiatric and behavioral health services across diverse patient populations
- Strong knowledge of psychiatric medications, treatment planning, and crisis intervention
- Proficiency with Electronic Health Records (EHR) and healthcare technology systems
- Understanding of healthcare compliance, confidentiality, and documentation standards
Salary Description
$100,000.00 - $160,000.00 Depending on Experience
