Our organization is expanding its behavioral health team and is seeking a <\/span><\/span>Psychiatric Mental Health Nurse Practitioner (PMHNP)<\/span><\/span><\/b> to support our partnered nursing homes throughout Saten Island<\/span><\/span><\/span><\/span><\/b>. We specialize in delivering high -quality, compassionate mental health services to older adults in long -term care settings. This position is ideal for a clinician who is passionate about geriatric psychiatry and dedicated to supporting the emotional and psychological wellness of aging populations.<\/span>
<\/span><\/span><\/p>
Conduct detailed psychiatric assessments and follow -up evaluations for elderly residents in nursing home environments.<\/span>
<\/span><\/span><\/p><\/li>
Create individualized treatment plans tailored to each patient’s mental health concerns and diagnostic profile.<\/span>
<\/span><\/span><\/p><\/li>
Provide ongoing psychiatric medication management and make clinical adjustments as needed.<\/span>
<\/span><\/span><\/p><\/li>
Work collaboratively with physicians, psychologists, social workers, and facility staff to ensure a coordinated, whole -person care approach.<\/span>
<\/span><\/span><\/p><\/li>
Deliver therapeutic interventions, including counseling and supportive psychotherapy.<\/span>
<\/span><\/span><\/p><\/li>
Guide patients and families by explaining diagnoses, treatment paths, and strategies for managing mental health conditions.<\/span>
<\/span><\/span><\/p><\/li>
Maintain clear, accurate, and compliant documentation within electronic health record systems.<\/span>
<\/span><\/span><\/p><\/li>
Participate in team conferences, interdisciplinary meetings, and quality improvement efforts within the organization.<\/span>

Active <\/span><\/span>New York State Nurse Practitioner license<\/span><\/span><\/b> with PMHNP certification.<\/span>

Ability to work independently while effectively managing a caseload across multiple facilities.<\/span>
<\/span><\/span><\/p><\/li>
Strong diagnostic abilities and familiarity with psychiatric medications and therapeutic approaches.<\/span>

Excellent communication and interpersonal skills, with the ability to build trust with patients, families, and care teams.<\/span>

Proficiency with EHR systems and basic computer applications.<\/span>

Salary range:<\/span><\/span><\/b> $140,000–$150,000 per year (commensurate with experience)<\/span>
