Position Summary:The Psychiatric Mental Health Nurse Practitioner (PMHNP) delivers compassionate, patient-centered psychiatric care across the lifespan. This role involves collaboration within a multidisciplinary care team to assess, diagnose, and treat mental health conditions, create individualized treatment plans, and support patients in managing their mental wellness. Key Responsibilities Patient Care - Perform comprehensive psychiatric assessments, including history, mental status exams, and diagnostic evaluations.
- Develop, implement, and revise individualized treatment plans.
- Prescribe and manage psychiatric medications in accordance with clinical guidelines and applicable regulations.
- Provide psychotherapy (individual, group, or family), as appropriate.
- Educate patients and families on diagnoses, treatment options, and preventive strategies.
Interdisciplinary Collaboration - Collaborate with primary care providers, social workers, and other team members to deliver integrated care.
- Actively participate in team meetings to review and adjust patient care plans.
- Serve as a resource for staff regarding behavioral health concerns.
Documentation - Maintain accurate, thorough, and timely documentation in the electronic health record (EHR) system in accordance with organizational and regulatory standards.
Quality Improvement - Engage in continuous quality improvement efforts to enhance clinical outcomes and service delivery.
- Stay informed on current best practices and evolving trends in psychiatric care.
Community Engagement - Support mental health education and outreach efforts to reduce stigma and promote awareness.
- Collaborate with community organizations to improve access to behavioral health resources.
Other Duties - Perform additional duties as assigned by the supervisor that are aligned with the mission and scope of this role.
