• Conduct detailed psychiatric evaluations and mental health assessments for residents in long -term care environments.<\/span>
  <\/span><\/span><\/p><\/li>

• Diagnose mental health conditions and develop individualized treatment plans tailored to each patient’s clinical needs.<\/span>
  <\/span><\/span><\/p><\/li>

• Oversee medication management, track patient response, and revise treatment strategies when necessary.<\/span>
  <\/span><\/span><\/p><\/li>

• Partner with an interdisciplinary team of psychologists, physicians, nurses, and social workers to ensure well -coordinated, whole -person care.<\/span>

• Deliver therapeutic interventions, including counseling and psychotherapy when appropriate.<\/span>
  <\/span><\/span><\/p><\/li>

• Educate patients and their families on psychiatric conditions, treatment expectations, medication compliance, and wellness practices.<\/span>

• Maintain thorough, accurate, and compliant documentation within the EHR system.<\/span>

  Candidate Requirements<\/span><\/span><\/b>

  • Active <\/span><\/span>New York State Nurse Practitioner license<\/span><\/span><\/b> with <\/span><\/span>PMHNP certification<\/span><\/span><\/b>.<\/span>

  • Ability to manage a caseload across multiple facilities independently.<\/span>
    <\/span><\/span><\/p><\/li>

  • Strong diagnostic skills and a solid understanding of psychiatric medications, therapies, and clinical best practices.<\/span>

  • Exceptional communication and interpersonal skills with a patient -centered approach.<\/span>
