  • Perform thorough psychiatric assessments and mental status evaluations for residents in nursing homes.<\/span>

  • Establish accurate diagnoses and design personalized treatment plans tailored to each patient's needs.<\/span>
    <\/span><\/span><\/p><\/li>

  • Provide ongoing medication management, making timely adjustments based on clinical progress.<\/span>
    <\/span><\/span><\/p><\/li>

  • Work collaboratively with physicians, psychologists, social workers, nursing staff, and other healthcare professionals to coordinate comprehensive care.<\/span>

  • Deliver supportive counseling and psychotherapy to patients and their families.<\/span>
    <\/span><\/span><\/p><\/li>

  • Educate residents and caregivers about psychiatric conditions, treatment options, and practical self -management strategies.<\/span>

  • Maintain precise and compliant documentation within the electronic health record (EHR) system.<\/span>

    Qualifications<\/span><\/span><\/b>
    <\/span><\/span><\/h2>

    • Active <\/span><\/span>New York State Nurse Practitioner license<\/span><\/span><\/b> with certification as a <\/span><\/span>Psychiatric–Mental Health Nurse Practitioner (PMHNP)<\/span><\/span><\/b>.

    • Ability to work independently while managing a consistent caseload.<\/span>

    • Strong diagnostic abilities and familiarity with psychopharmacology and evidence -based treatment approaches.<\/span>

    • Excellent communication skills and the ability to build rapport with patients, families, and care teams.<\/span>

    • Proficiency in electronic health records and basic computer applications.<\/span>
