  • Perform thorough psychiatric evaluations and ongoing mental health assessments for residents in long -term care settings.<\/span>
    <\/span><\/span><\/p><\/li>

  • Formulate tailored treatment plans that reflect each patient’s specific mental health needs and diagnoses.<\/span>
    <\/span><\/span><\/p><\/li>

  • Oversee and adjust medication regimens based on clinical response and best -practice guidelines.<\/span>
    <\/span><\/span><\/p><\/li>

  • Partner with interdisciplinary professionals—including physicians, therapists, social workers, and nursing staff—to ensure coordinated and holistic patient care.<\/span>
    <\/span><\/span><\/p><\/li>

  • Deliver counseling and psychotherapy to residents and support families in understanding care plans.<\/span>
    <\/span><\/span><\/p><\/li>

  • Provide education to patients and caregivers regarding mental health conditions, available treatments, and ongoing self -care strategies.<\/span>

  • Maintain compliant, accurate, and timely documentation within electronic health records.<\/span>

  • Engage in case conferences, team meetings, and quality improvement efforts to support clinical excellence.<\/span>

    Qualifications<\/span><\/span><\/b>
    <\/span><\/span><\/div>

    • Active <\/span><\/span>New York State Nurse Practitioner license<\/span><\/span><\/b> with certification as a <\/span><\/span>PMHNP<\/span><\/span><\/b>.<\/span>
      <\/span><\/span><\/p><\/li>

    • Ability to work independently in a fast -paced clinical environment and manage a diverse caseload.<\/span>
      <\/span><\/span><\/p><\/li>

    • Strong skills in psychiatric diagnosis, treatment planning, psychopharmacology, and patient assessment.<\/span>
      <\/span><\/span><\/p><\/li>

    • Excellent communication and relationship -building abilities with patients, families, and colleagues.<\/span>

    • Proficiency in EHR systems and general computer use.<\/span>
