CC017225 Patient Family Centered CareCoordinates health education programs and training of community partners. Maintains resources for projects, team development, team meetings coordination, data entry and analysis. Also maintains up-to-date knowledge of advances and understanding of injury-related risks and protective factors.
1. Administers programs under direction, including maintenance of best practice knowledge, participant recruitment, training development, and event logistics.
2. Creates community engagement by identifying key stakeholders, building relationships, and participating in or leading community events/activities.
3. Conducts program evaluation, quality improvement and data management.
4. Represents AC in partnerships addressing mechanism-related task forces and coalitions.
5. Performs other duties as assigned.
