Job Summary:
The Program Manager, Oncology Navigation Services is responsible for the strategic development, implementation, management, and evaluation of oncology navigation programs across the cancer care continuum. This role oversees patient navigation services, ensuring timely access to care, reduction of barriers to treatment, improved patient outcomes, and an exceptional patient experience. This role helps patients navigate the healthcare system from diagnosis through treatment, survivorship, and follow-up care by facilitating access to services, reducing barriers to care, coordinating appointments, and connecting patients with internal and community resources. Working collaboratively with physicians, nurses, social workers, financial counselors, and other members of the multidisciplinary team, the program manager promotes a seamless, patient-centered experience while supporting timely access to care and treatment compliance.
Essential Duties and Responsibilities:
Program Leadership and Development
Develop, implement, and manage oncology navigation programs aligned with organizational goals and accreditation standards.
Establish strategic initiatives to improve patient access, care coordination, and clinical outcomes.
Lead program expansion efforts for disease-specific navigation, survivorship, and supportive care services.
Monitor program performance and implement evidence-based improvements.
Navigation Operations Management
Oversee daily operations of oncology navigation services.
Standardize navigation workflows, processes, and documentation practices.
Ensure timely patient referrals, scheduling, and coordination across the care continuum.
Collaborate with clinical teams to streamline transitions from diagnosis through survivorship or end-of-life care.
Team Leadership
Provide coaching, mentoring, and professional development opportunities.
Establish performance goals and conduct evaluations.
Foster collaboration among multidisciplinary oncology teams.
Patient-Centered Care Coordination
Ensure patients receive timely education, resources, and support throughout treatment.
Identify and address barriers to care, including financial, transportation, psychosocial, cultural, and health literacy challenges.
Support patient engagement initiatives and shared decision-making processes.
Promote equitable access to oncology services.
Quality Improvement and Outcomes Management
Develop and monitor key performance indicators (KPIs) related to navigation services.
Analyze patient outcomes, navigation metrics, and operational data.
Lead quality improvement initiatives to enhance patient satisfaction and care coordination.
Prepare reports and presentations for leadership and cancer committees.
Accreditation and Regulatory Compliance
Support compliance with accreditation standards and quality requirements established by organizations such as Commission on Cancer and other oncology regulatory bodies.
Coordinate documentation and reporting related to patient navigation standards.
Participate in cancer committee meetings, accreditation surveys, and program reviews.
Community Outreach and Resource Development
Develop partnerships with community organizations and support services.
Promote awareness of oncology navigation services among patients, providers, and referral sources.
Coordinate patient education programs and survivorship initiatives.
Identify resources that address social determinants of health impacting oncology patients.
Financial and Resource Management
Assist with budget development and resource allocation.
Monitor staffing needs and program productivity.
Evaluate opportunities for program growth and sustainability.
Support grant applications and funding initiatives when applicable.
Requirements and Qualifications:
Education
Bachelors degree in Nursing, Healthcare Administration, Public Health, Social Work, or related field required.
Masters degree preferred (MSN, MHA, MPH, MBA, or related healthcare discipline).
Preferred Certification as a Patient Navigator
Professional Certification in navigation will be required within 12-18 months of appointment
Experience
Minimum 3-5 of experience in healthcare, patient navigation, care coordination, case management support, patient services, customer service, or a related healthcare setting.
Oncology Experienced Proffered
Experience working directly with patients, families, and interdisciplinary healthcare teams.
Strong organizational, communication, and problem-solving skills.
Proficiency with electronic medical record (EMR) systems and Microsoft Office applications.
Knowledge, Skills, and Abilities
Comprehensive understanding of oncology care delivery and patient navigation principles
Strong leadership and team management skills
Program development and project management expertise
Knowledge of cancer program accreditation standards such as ASCO and Commission on Cancer Standards
Data analysis, reporting, and performance improvement experience
Excellent communication, collaboration, and stakeholder engagement skills
Understanding of healthcare reimbursement and financial assistance programs
Ability to address health equity and social determinants of health challenges
